I received this from Apple at one point. I'm not sure if it's still valid though:

You can set up an account that will not require a payment type by going into the
app store (not signed in to any apple id) and selecting a free app to download.
it will then prompt you to create an account. You will need to create the
account using an email address that is not already attached to an apple id

It will then allow you to select the "none" option for payment type

As far as managing the devices, there is some built-in stuff from Apple, but
it's somewhat limited. We use and sell Centrify for SaaS for our iPads which
allows tracking, applying policy, remote-lock. SSO and remote-wipe functionality:
