It overlooks one important detail: These days, in most cases, you
must be a subscriber to a list in order to post a message on it.
This tends to exclude the overwhelming majority from participating in
cross-posted threads, except on whichever list where they are already
subscribed. Actually for this reason and others, cross-posting is
itself generally considered poor netiquette. This tends to confound
the argument in favor of reply-all.
I do tend to forget to list-reply due to work habits, but when dealing
with non-work mailing lists, I can't remember the last time I ever
CC'd someone who wasn't subscribed on a list post; I could probably
count the number of times I've posted a message to multiple mailing
lists simultaneously on my fingers.
