It sounds like an intermittent hardware issue. Have you tried re-seating
the cables and cards?

We have a Sun T2000 that has been giving us this error every a couple of months
or so. It's not always the same disk target that has the problem. It happene
d on two different targets (one that is RAID'ed and one that is not) We do not
believe it's actually a bad disk. I found this old post but nothing helped:
